We are seeking a dedicated Registered Nurse to contribute to the Primary Care Network. Relocation assistance may be provided, and there are opportunities for student loan forgiveness at both provincial and federal levels.
The ideal candidate will work within the Primary Care Network, providing essential nursing services and support to patients in a collaborative environment.
Key Responsibilities:
Evaluate, plan, implement, and assess nursing care for patients and their families. Collaborate with Primary Care Providers according to the established care delivery model. Utilize health promotion and chronic disease management strategies to conduct thorough assessments and address care plans. Work alongside interdisciplinary teams to ensure continuity of care and facilitate transitions. Serve as a nursing resource, employing critical thinking and problem-solving skills to enhance patient care.About Primary Care Networks:
Primary Care Networks (PCNs) are integral to the provincial Primary Health-Care Strategy, fostering local partnerships among Health Authorities, Divisions of Family Practice, and First Nations communities. These networks consist of multidisciplinary teams, including nurses, pharmacists, and allied health professionals, who collaborate with family physicians and nurse practitioners to deliver comprehensive care.
PCNs aim to improve patient access to health services and enhance the capacity of existing healthcare providers. By adopting a team-based approach, these networks ensure that patients receive a holistic view of their healthcare needs, facilitating easier access to a range of services.
Qualifications:
Education & Experience
Current registration as a Registered Nurse with the BC College of Nurses and Midwives (BCCNM). Minimum of one (1) year of relevant clinical experience or an equivalent combination of education and training. Valid driver's license and access to a vehicle.Skills & Competencies:
Strong critical thinking abilities within the scope of patient care. Knowledge of primary care principles and patient-centered care models. Familiarity with professional regulations and standards relevant to community health nursing. Competence in evidence-based clinical practices related to primary care. Understanding of provincial regulations and guidelines pertaining to community health. Ability to provide culturally safe care across diverse populations. Effective communication and conflict resolution skills. Proficiency in utilizing technology for patient care documentation.
