Home and Community Care Support Services is seeking a seasoned program manager with a comprehensive grasp of engagement methodologies, strategies, and the technical expertise necessary for developing, implementing, and assessing engagement initiatives for both internal and external stakeholders. The Community Engagement Specialist will lead efforts to enhance patient involvement within care networks and throughout the community, including advisory groups, feedback collection, experience-based co-design, and evaluations centered on patient needs.

Key Responsibilities:

In response to engagement inquiries from colleagues, facilitate various outreach activities (including virtual sessions and individual outreach) with individuals who have lived experiences to encourage their involvement and gather insights for program development and quality enhancement. Organize meetings, prepare presentation materials, and deliver presentations to community organizations and partner entities, supporting diverse engagement initiatives. Gather, analyze, and present feedback data from engagement activities. Identify and enlist community members with lived experiences for participation in engagement forums and events across the region. Plan, coordinate, and facilitate engagement events and activities, such as focused discussions. Establish formal and informal communication channels with patients, families, and caregivers to obtain feedback on healthcare performance and improve system processes. Recognize gaps in planning and design where patients, families, and caregivers have not had opportunities for input, and assist colleagues in creating avenues for feedback. Build and maintain relationships with service providers and both internal and external partners as needed. Promote broader engagement with Indigenous, Francophone, and underrepresented communities.

Required Qualifications:

Degree, Certificate, or Diploma in community or social services, healthcare, administration, or a related field (or equivalent combination of education and experience). Three (3) to five (5) years of relevant experience in program management. Strong understanding of the mission, vision, values, and operational frameworks of Home and Community Care Support Services, as well as the broader healthcare landscape. Proven ability to engage with a diverse group of providers and community members through a health equity lens. Excellent communication and facilitation skills. Ability to interact with tact and diplomacy to build rapport with patients, families, caregivers, staff, and partners. Proficient in analyzing information, problem-solving, and making informed decisions. Awareness of ethnic, spiritual, linguistic, familial, and cultural differences. Solid understanding of engagement strategies and technical skills necessary for planning, executing, and evaluating engagement initiatives. Knowledge of current trends in engagement practices, including both in-person and virtual sessions. Familiarity with engagement policies and patient-centered care philosophies. Preferred Qualifications:

Professional experience in an engagement program, particularly within the healthcare sector. Personal experience navigating various services and supports within the healthcare system. Experience working with Indigenous, Francophone, racialized, Two-Spirit and LGBTQIA+, disabled, and other underserved communities. Bilingualism (English/French) is considered an asset.
