Job Title: Community Engagement Specialist

About the Role:

We are seeking a highly skilled Community Engagement Specialist to join our team at Home and Community Care Support Services. As a Community Engagement Specialist, you will play a key role in engaging with diverse community members and healthcare providers to improve health outcomes and patient experiences.

Key Responsibilities:

  • Conduct engagement activities with community members and healthcare providers to gather input and feedback on healthcare services and programs.
  • Facilitate meetings and presentations to community groups and partner organizations to promote healthcare services and programs.
  • Collect, analyze, and report engagement feedback data to inform healthcare service improvements.
  • Identify and recruit community members with lived experience to participate in engagement forums and activities.
  • Plan, coordinate, and facilitate engagement events and activities, including focus groups and community meetings.
  • Develop formal and informal channels of communication with patients, families, and caregivers to receive feedback on healthcare services and improve healthcare processes and tools.
  • Establish relationships and communicate with service providers and external partners as necessary.
  • Support broader engagement with Indigenous, Francophone, and underrepresented communities.

Requirements:

  • Degree, certificate, or diploma in community or social services, healthcare, general administration, or a related field (or equivalent combination of education and experience).
  • Three to five years of related and relevant experience providing program coordination.
  • Good understanding of Home and Community Care Support Services' mission, vision, values, mandate, client service delivery frameworks, and methods.
  • Demonstrated ability to engage and relate with a diverse group of providers and community members within a health equity lens.
  • Strong communication and facilitation skills.
  • Tact and diplomacy to interact and build rapport with various patients, families, caregivers, staff, and partners.
  • Ability to analyze information, problem-solve, and make good decisions.
  • Overall understanding of ethnic, spiritual, linguistic, familial, and cultural differences.
  • Solid understanding of engagement concepts, strategies, and technical skills required in planning, execution, and evaluation of engagement plans for internal and external audiences.
  • Knowledge of current trends in engagement planning and tactics, including in-person and virtual engagement sessions and other channels.
  • Good understanding of engagement policies and procedures, and patient- and family-centred care philosophy and/or frameworks.

Salary: $67,765 to $80,915 annually.
