We are seeking a highly skilled and dedicated Registered Nurse to join our team at Fraser Health. As a Registered Nurse, you will play a vital role in providing high-quality patient care and contributing to the delivery of exceptional healthcare services.
Key Responsibilities- Assess patients and collect information from various sources to develop effective care plans.
- Collaborate with patients, families, and caregivers to promote person-centered care and ensure their needs are met.
- Provide direct patient care by assessing, planning, providing, and evaluating nursing interventions.
- Participate in the care and discharge planning process for patients, ensuring comprehensive care planning and delivery.
- Coordinate nursing care for patients, set priorities, assign and delegate tasks to support staff, and make referrals to the interprofessional care team as needed.
- Collaborate with members of the interprofessional care team to identify and resolve issues, adjust care plans as required, and address changes in patient condition.
- Teach patients and families/caregivers about illness prevention, health restoration, and health maintenance to support their health care goals.
- Assess learning needs of patients and families/caregivers, provide information and demonstration of care activities to support self-care and the discharge plan.
- Document assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Ability to complete initial and ongoing patient assessment (clinical and diagnostic reasoning) and provide nursing care through appropriate/prescribed technical, therapeutic, safety-type interventions.
- Ability to communicate effectively with patients, families/caregivers, the public, medical staff, and members of the interprofessional care team using verbal, written, and electronic communication means.
- Ability to promote person-centered care that demonstrates empathy for patients and families/caregivers, sensitivity to diverse cultures and preferences, and patient advocacy.
- Ability to integrate and evaluate data from multiple sources to problem-solve effectively.
- Ability to teach and instruct patients and families/caregivers about topics essential to their health care and well-being.
- Ability to collaborate with and promote cooperation among members of the interprofessional care team.
- Ability to use factual information, prior learning, and basic principles and procedures to support nursing decisions and actions with relevant research-based evidence/evidence-informed practice.
