Job Summary
Vancouver Coastal Health is seeking a skilled Nurse to join our team as an Intake and Transition Nurse in the Primary Care department. As a key member of our team, you will be responsible for conducting intakes and transitions for clients, providing clinical guidance and support, and coordinating discharge and transitions as needed.
Key Responsibilities
- Conduct intakes and transitions for clients, assessing their needs and developing individualized plans to ensure seamless care.
- Provide clinical guidance and support to clients, families, and healthcare teams to promote optimal health outcomes.
- Coordinate discharge and transitions, ensuring a smooth transition of care for clients.
- Monitor client connection to clinic and/or 3rd parties, ensuring timely and effective communication.
- Assess and communicate clinic capacity to accept new referrals, ensuring efficient use of resources.
- Establish and enhance relationships with community resources and healthcare teams to promote collaborative care.
- Develop and implement transition plans for new referrals, facilitating interdisciplinary care conferences and maintaining accurate records.
Requirements
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two (2) years recent related experience in the designated clinical program area or an equivalent combination of education, training, and experience.
- Broad knowledge of nursing theory and practice within a client and family-centered care model.
- Knowledge of provincial acts, regulations, and program policies and guidelines related to home and community care.
- Ability to operate related equipment and maintain accurate records.
