Vancouver Coastal Health is seeking an experienced Intake and Transition Nurse to join the Primary Care team at Raven Song Community Health Centre in Downtown East Side (DTES), Vancouver BC.
Key Responsibilities- Conduct initial client assessments and interventions in a client-centered care model.
- Provide competent and culturally safe care in a variety of settings and with diverse populations.
- Collaborate with interdisciplinary teams to develop transition plans for new referrals and initiate and facilitate care conferences.
- Maintain accurate client records and provide data and statistics to support quality improvement initiatives.
- Participate in team meetings and continuing education to enhance skills and knowledge.
- Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two (2) years recent related experience in the designated clinical program area or an equivalent combination of education, training, and experience.
- Broad knowledge of nursing theory and practice within a client and family-centered care model.
- Knowledge of provincial acts, regulations, and program policies and guidelines related to home and community care.
