Job Summary:

The Group Health Centre is seeking a highly skilled and dedicated Registered Dietitian to join our team. As a key member of our healthcare team, the successful candidate will be responsible for providing comprehensive nutrition assessment, care planning, and education to patients and families in both group and individual settings.

Key Responsibilities:

• Collaborate with the interdisciplinary healthcare team to develop and implement patient care plans
• Conduct comprehensive nutrition assessments and provide personalized education to patients and families
• Develop and implement nutrition education programs for patients and families
• Participate in quality improvement initiatives and contribute to the development of standards and policies

Requirements:

• Registered Dietitian with a degree in Human Applied Nutrition or equivalent
• Active membership in the Ontario College of Dietitians in good standing
• Excellent interpersonal and communication skills
• Ability to work effectively as a member of an interdisciplinary team

Preferred Qualifications:

• Certified Diabetes Educator (CDE) Certificate
• Experience working with patients with diabetes
