Job Summary:
The Group Health Centre is seeking a highly skilled and dedicated Registered Dietitian to join our team. As a key member of our interdisciplinary healthcare team, the successful candidate will be responsible for providing comprehensive nutrition assessment, care planning, and education to patients and families in both group and individual settings.
Key Responsibilities:
- Conduct comprehensive nutrition assessments and develop personalized care plans for patients and families.
- Provide education and support to patients and families on nutrition-related topics, including diabetes management and healthy eating.
- Collaborate with the interdisciplinary healthcare team to ensure seamless care coordination and communication.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures related to nutrition and dietetics.
Requirements:
- Registered Dietitian with a degree in Human Applied Nutrition or equivalent, with active membership in the Ontario College of Dietitians in good standing.
- Excellent interpersonal and communication skills, with the ability to work effectively in a team environment.
- Demonstrated ability to work with patients and families from diverse backgrounds and with varying levels of health literacy.
- Knowledge of nutrition therapy and experience working with patients with diabetes and other chronic conditions.
