TRAINING OPPORTUNITY

In the event that there are not enough qualified applicants for this position, Vancouver Coastal Health (VCH) may provide an 8-week training program. A cover letter and resume are necessary. Candidates who accept this training opportunity commit to working with VCH in the relevant nursing area for a minimum of 18 months following the completion of the training. This commitment allows candidates to consolidate and apply the skills gained in the specialty area supported by VCH. Should a candidate leave the designated nursing area before the 18-month period, VCH will seek to recover the training costs.

Salary

The salary range for this role is CAD $41.42/Hr. - CAD $55.91/Hr.

Job Overview

Vancouver Coastal Health (VCH) is seeking a Community Health Nurse.

VCH is looking for Registered Nurses to become part of the Home Health team. This role involves working closely with clients and their families to set safe, realistic, and achievable goals through a comprehensive nursing process within an interprofessional team model.

Responsibilities of the Community Health Nurse include:

Collaborating with clients and families to assess, plan, coordinate, implement, evaluate, and transition nursing and related services. Evaluating service eligibility and facilitating the creation and adjustment of care plans. Providing direct care and care management to empower clients in managing their own health and navigating community services. Consulting with other system partners and integrating current evidence into practice.

Required Qualifications

Education & Experience

Current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). At least one year of recent clinical experience in a relevant area, particularly with complex clients/patients with chronic health conditions, or an equivalent combination of education and experience. A valid BC Driver's License is necessary as local travel may require the use of a vehicle or other transportation methods.

Knowledge & Skills

Strong understanding of community health nursing principles and practices within a client-centered care model. Ability to provide trauma-informed, culturally safe care through thorough client assessments and interventions. Critical thinking skills to assess safety and risk in various home environments and effectively solve problems. Proficient in managing psychosocial issues and employing conflict resolution strategies related to care goals. Capacity to prioritize and manage multiple clients while promoting cooperation and innovation in care delivery. Resilience in adapting to unexpected situations while advocating for client-centered care. Collaborative skills to work effectively within an interdisciplinary team to assess and manage diverse health challenges. Competence in documenting client information accurately in electronic medical records. Ability to maintain confidentiality regarding client information and communication. Effective communication skills to build trust and maintain professional boundaries with clients and healthcare professionals. Skills in teaching and facilitating client engagement in their care planning and self-management. Knowledge of relevant provincial regulations and community resources for client support. Understanding of health management across acute, chronic, and palliative care settings. Ability to organize and prioritize work effectively. Proficiency in CPR techniques and nursing treatment equipment. Familiarity with BCCNM standards of practice and clinical guidelines. Physical capability to perform the duties of the position.

Work hours, including days off and work areas, may vary based on operational needs and collective agreements.

As per current Public Health Orders, all employees of Vancouver Coastal Health must be fully vaccinated for COVID-19 or have received the most recent updated vaccine. Proof of vaccination status will be required.
