About the Role:

Traverse Independence is seeking a highly skilled and compassionate Community Facilitator to join our team. As a Community Facilitator, you will play a vital role in supporting individuals with acquired brain injuries (ABI) to live independently in the community.

Key Responsibilities:

  • Provide support to individuals in their homes or community settings across the County of Wellington, including Rural Wellington.
  • Build trusted relationships with clients and support them to work on independence in the community.
  • Collaborate with a multidisciplinary team, including regulated staff such as ABI specialized psychiatrists, an Occupational Therapist, Behaviour Therapist, and an Intensive Case Coordinator.
  • Provide support and guidance to clients to help them achieve their goals, improve their quality of life, and cope with their day-to-day activities.
  • Utilize strong organizational skills, interpersonal skills, and communication skills to effectively work with clients and the team.

Requirements:

  • Community College Diploma in Social Services, Developmental Services, Health Care, or related discipline.
  • Demonstrated experience in behaviour management skills is a benefit.
  • Experience with people with disabilities such as mental health and/or addictions is a great asset but not necessary.
  • Strong utilization and understanding of M365 - Teams/SharePoint.
  • Demonstrated organizational skills, interpersonal skills, and communication skills.
  • Ability to work as a member of a multidisciplinary team.
  • Must have a valid driver's license and access to a reliable vehicle with proof of $1,000,000 liability insurance and driver's abstract acceptable to Traverse Independence.
