Position Overview

The Client Care Coordinator plays a pivotal role within BC Mental Health & Substance Use Services, focusing on the delivery of care to clients in correctional settings. This position emphasizes the importance of accessible healthcare and aims to foster a supportive environment for individuals seeking treatment.

Key Responsibilities

  • Lead and coordinate client care initiatives, ensuring innovative and effective service delivery through collaboration with interdisciplinary teams.
  • Support staff in implementing care standards and guidelines, while monitoring adherence to established protocols and quality assurance measures.
  • Participate in initial client evaluations and contribute to care planning discussions, assessing client needs and prioritizing care requirements.
  • Facilitate communication among healthcare team members, providing consultation and guidance on complex cases to enhance treatment outcomes.
  • Build and maintain therapeutic relationships with clients, empowering them to engage in their care and improve their overall well-being.

Qualifications

  • Graduation from an accredited School of Nursing with current registration as a Registered Nurse or Registered Psychiatric Nurse.
  • Completion of relevant specialty nursing programs and a minimum of three years of recent experience in a clinical setting.
  • Valid driver's license and access to a vehicle may be required.
  • Successful completion of Occupational First Aid certification.

  • Proficient in evidence-based nursing practices and therapeutic communication.
  • Ability to lead teams and conduct comprehensive clinical assessments.
