**Job Summary**

The Indigenous Patient Site Coordinator is a key role within the WRHA Corporate team, responsible for ensuring the efficient and effective functioning of the Indigenous Health Services program on site. This position requires a strong understanding of Indigenous culture and community needs, as well as excellent communication and leadership skills.

**Key Responsibilities**

  • Plan and coordinate health and human services affecting Indigenous patients, working closely with healthcare professionals and community stakeholders.
  • Participate in Regional WRHA Indigenous Health Services Program activities, contributing to the development and implementation of program strategies.
  • Provide administrative support to the Program Director - Indigenous Health Services, ensuring seamless day-to-day operations.
  • Collaborate with other departments and agencies to identify and address the needs of Indigenous patients and families.
  • Develop and maintain relationships with community partners, healthcare providers, and other stakeholders to ensure effective service delivery.

**Requirements**

  • Minimum of three (3) years supervisory/administrative experience working with the Indigenous community in a healthcare related field.
  • Bachelor's degree in Social Work or relevant social science discipline.
  • Knowledge of computer systems and proficiency in the use of current and related software applications.
  • Knowledge of and working familiarity with services of government departments and agencies and organizations providing assistance to Indigenous people.
  • Knowledge and understanding of Indigenous culture.
  • Ability to interact well with other healthcare professionals, all levels of staff, and patients and families.
  • Demonstrated planning and problem-solving skills.
  • Demonstrated leadership abilities.
  • Excellent oral and written communication skills.
  • Fluent in English.
  • Mature, professional manner.
