Wellness Nurse

At Sunrise Senior Living, we are committed to providing high-quality care and services to our residents. As a Wellness Nurse, you will play a vital role in promoting the overall well-being of our residents.
Responsibilities:- Conduct regular wellness visits for all residents to assess their health and wellness needs.
- Develop and implement individualized care plans to meet the unique needs of each resident.
- Collaborate with the Resident Care Director to ensure that residents receive the highest quality care and services.
- Monitor and document resident health and wellness information, including vital signs and medication administration.
- Communicate effectively with residents, families, and healthcare providers to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance the overall quality of care and services provided.
- Graduate of an accredited nursing program.
- Current state license as a Registered Nurse (RN).
- Minimum of one year of experience in a healthcare setting, preferably in senior care.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Proficiency in computer skills and electronic medical records.
