In alignment with the established vision and values of the Northern Health Authority, the Community Care Nurse operates in accordance with the professional practice standards and ethical guidelines set forth by the BC College of Nurses and Midwives (BCCNM), while adhering to a patient and family-centered care approach.
Responsibilities include assessing, planning, organizing, implementing, delivering, and evaluating nursing and related services for clients in a community context by identifying client needs, developing patient care plans, administering treatments, and offering support and health education as necessary.
The nurse collaborates with clients, caregivers, physicians, healthcare providers, and other organizations to coordinate long-term care, nursing services, home support, or additional service delivery to clients.
Primary Care Networks consist of partnerships among the Divisions of Family Practice representing BC's family physicians, regional health authorities, the First Nations Health Authority (FNHA), and local community partners, alongside other community providers such as nurse practitioners.
All partners are working collaboratively in their local communities to enhance the primary and community care system for improved outcomes for patients and healthcare providers.
Work Schedule: Monday to Friday, Days, 08:00 to 16:00
Compensation:
The wage is: $59.52/hour

- Completion of gerontology content, post-basic course in wound care management, or an equivalent combination of training and experience.
- Post-basic course in wound care management and gerontology preferred.
- Valid BC Driver's License.
- Assessment and Intervention – Proven ability to conduct initial and ongoing client assessments and provide nursing care through appropriate interventions.
- Communication – Effective communication skills with clients, families, the public, medical staff, and interdisciplinary team members using various communication methods.
- Critical Thinking – Ability to integrate and evaluate relevant data to solve problems effectively.
- Human Caring and Relationship-Centered Practice – Ability to promote client-focused care that respects diverse cultures and preferences, advocating for clients and addressing social justice concerns.
- Teaching – Capability to instruct clients and others on essential health care topics.
- Management – Proficient in managing time and resources, fostering cooperation among relevant parties, and collaborating across disciplines.
- Leadership – Encourages staff morale, cooperation, and innovation while implementing policies and promoting professional development.
