Registered Nurse/Registered Psychiatric Nurse, Hospice Residence

Fraser Health is seeking a skilled Registered Nurse/Registered Psychiatric Nurse to join our Hospice Residence team. As a member of our multidisciplinary team, you will provide direct nursing care to patients, assessing their physical, emotional, spiritual, and psychosocial needs, and developing and implementing care plans to meet their unique needs.
Responsibilities- Assess and plan nursing care for patients, implementing and evaluating interventions to achieve optimal outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Collaborate with interdisciplinary teams to develop and implement care plans that address the physical, emotional, and spiritual needs of patients.
- Document patient information accurately and timely, using electronic health records and other tools as needed.
Graduation from an approved School of Nursing and/or Psychiatric Nursing is required. Two years of recent, related medical nursing experience in an acute care facility, including one year of recent experience in a hospice/palliative care environment, or an equivalent combination of education, training, and experience is preferred. Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM) is required.
