Job Title: Community and Social Services Worker

We are seeking a highly motivated and detail-oriented Community and Social Services Worker to join our team at Persons Community Solutions LTD.

Job Summary:

The successful candidate will be responsible for providing administrative and office support, as well as liaising with other social services agencies and healthcare providers to ensure seamless client care.

Key Responsibilities:
  • Administrative and office activities, including data entry and record-keeping
  • Liaise with other social services agencies and healthcare providers to ensure effective client care
  • Conduct criminal record checks and maintain accurate records
  • Develop and maintain strong interpersonal skills to effectively communicate with clients and colleagues
  • Assist with client intake, assessment, and case management
  • Participate in team meetings and contribute to the development of client-centered plans
Requirements:
  • High school diploma or equivalent required; post-secondary education in a related field an asset
  • Previous experience in a social services or administrative role an asset
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong organizational and time management skills
