About the Role

The Care Coordinator is a vital member of our team at pt Health, responsible for delivering exceptional patient care and administrative support. This role requires a strong relationship builder who can foster a positive experience for our patients, staff, funders, and referral sources.

Key Responsibilities

  • Patient Intake and Coordination
    • Efficiently gather and document patient information, ensuring accurate and detailed records.
    • Coordinate patient scheduling, appointments, and treatment plans.
  • Administrative Support
    • Provide administrative assistance to the Care Coordinator Manager and Clinic Director.
    • Manage data entry, billing, and invoicing processes.
  • Customer Service and Communication
    • Deliver exceptional customer service to patients, staff, and external customers.
    • Communicate effectively with patients, staff, and external partners.
  • Qualifications and Competencies
    • Education and Experience
      • High School Diploma, G.E.D., or equivalent.
      • Experience in a healthcare or administrative setting an asset.
    • Skills and Abilities
      • Excellent customer service and communication skills.
      • Strong organizational and time management skills.
      • Ability to work in a fast-paced environment and prioritize tasks effectively.
