Community Support Worker
Position Overview:

Key Responsibilities:
- Engage in the assessment and planning processes to create individualized Support Plans for participants with diverse needs. Document and execute these plans while collaborating with the program manager for evaluations.
- Assess the needs of participants and collaborate with families to develop short-term strategies that address those needs.
- Assist participants in achieving independence or semi-independence by providing guidance in daily living skills, social interactions, and behavior management. Facilitate activities that promote personal hygiene, basic cooking, financial literacy, shopping, and home safety.
- Support the intake process by ensuring all assessments, registrations, and necessary documentation are completed prior to program commencement.
- Identify and manage potential emergency situations, including behavioral challenges, to ensure the safety of participants and the community. Report any issues to the supervisor.
- Administer medications to clients in accordance with established protocols.
- Maintain health and safety standards throughout all activities.
- Provide ongoing feedback and support to participants and their families.
- Keep detailed records of participants' daily activities and progress, including transition reports and case notes, ensuring all documentation is accurate and current.
- Transport participants as needed for various activities.
- Perform additional related duties as required.
Qualifications:
- A diploma in a relevant human or social services field, along with one year of recent related experience, or an equivalent combination of education and experience.
- A Class 4 driver's license or a commitment to obtain one promptly.
- Successful completion of the agency's screening process, including a criminal record check.
This position is open to all qualified candidates.
Hours: On-call availability during weekdays.
Compensation: $25.95 per hour.
