Position Overview: Vancouver Coastal Health is seeking a Registered Nurse to become part of the South East Urgent Primary Care Centre (UPCC) team.

Key Responsibilities:

  • Triage, assess, plan, implement, and evaluate nursing care for patients and families requiring urgent attention.
  • Establish and nurture partnerships with primary care providers in alignment with the local Community of Care (COC) service delivery model.
  • Utilize approved triage standards while applying critical thinking skills within safe patient care limits.
  • Implement chronic disease management, palliative care, health management, and self-care management principles to conduct assessments and address care plan variances.
  • Serve as a resource for the interdisciplinary team by providing insights based on nursing theory and practice.

About Urgent and Primary Care Centres:

Urgent and Primary Care Centres (UPCC) cater to individuals with unexpected, non-life-threatening conditions requiring medical attention within 12-24 hours. This includes issues such as sprains, cuts, infections, mild asthma attacks, and less severe childhood illnesses and injuries.

The multidisciplinary healthcare team comprises general practitioners, nurse practitioners, registered nurses, and social workers, enhancing community service delivery.

Qualifications:

  • Two (2) years of recent clinical experience relevant to the designated clinical area, or an equivalent combination of education, training, and experience.
  • Proficiency in applying critical thinking skills within safe patient care limits.
  • Demonstrated knowledge and skills in primary care, including patient and family-centered care models.
  • Understanding of Community Health Nurse of Canada (CHNC) competencies and standards of practice.
  • Knowledge of provincial acts, regulations, and guidelines related to home, community, and primary care.
  • Experience in providing culturally safe care to diverse populations.
  • Familiarity with Trauma Informed Practice, Harm Reduction, and Recovery Oriented Care.
  • Ability to work independently and prioritize care effectively.
  • Knowledge of adult education principles and the ability to instruct and support patients and families.
  • Demonstrated skill in CPR techniques.
