Primary Care Nurse

Providence Health Care is seeking a skilled Primary Care Nurse to join our team at the BC Centre for Excellence in HIV/AIDS' Research & Innovation Centre.
About the RoleThe Primary Care Nurse will be a member of an interprofessional healthcare team, working with clients/patients in the Downtown Eastside. The successful candidate will have a strong background in primary care, HIV, addictions, and mental health, with experience working with diverse marginalized populations.
Key Responsibilities- Conduct assessments and develop care plans in collaboration with the healthcare team
- Triage for physical health and psychosocial needs
- Perform primary care nursing duties, including wound care, addiction assessments, harm reduction, HIV and HCV testing, phlebotomy, diabetes care, pain management, and pregnancy and urine drug tests
- Document information, including assessments, plan of care, patient/client status, and progress
- Provide continuity of care by liaising with the healthcare team and implementing clinical interventions
- Registration with the BC College of Nurses and Midwives (BCCNM) as a practicing RN registrant or RPN registrant
- Two years' recent related experience working with diverse marginalized populations in healthcare settings or community in a primary care, HIV, addictions, and/or mental health setting
- Completion of the Point of Care HIV Testing course and successful completion of the BCCDC Immunization certificate and Narcan Certification
- Demonstrated experience and/or successful completion of recent education/training in performing venipuncture
