Access Navigator, Provincial Preschool Autism Service Coordinator

We are seeking a highly skilled and compassionate Access Navigator to join our Provincial Preschool Autism Service team. As a key member of our clinical intake and care coordination stream, you will be the first point of contact for families contacting our service. Your primary responsibility will be to explore child and family needs, match them with available services, and provide navigation and coordination support throughout their preschool years.
Key Responsibilities- Provide initial contact and assessment for families seeking our services
- Collaborate with clinicians and administrative staff to ensure a coordinated continuum of care
- Develop and maintain professional relationships with families throughout their preschool years
- Support access to school-age diagnostic services as needed
- Participate in planning, development, and ongoing evaluation/quality improvement
- Bachelor of Social Work or equivalent degree
- Registration with the Nova Scotia College of Social Workers or applicable College
- Minimum of two years related clinical experience
- Three to five years' experience in intake and/or client navigation preferred
- Experience in pediatric settings and specifically autism preferred
- Knowledge of IWK, Nova Scotia Health, Autism Nova Scotia, Hearing and Speech Nova Scotia, and community resources
- Ability to effectively conduct virtual and telephone-facilitated intake
- Ability to match criteria for clinical streams to identify patient and family needs
- Ability to accurately enter data and use databases and patient information systems
- Able to work in a collaborative and consultative manner as part of an interprofessional team
- Demonstrated clinical competence and experience with the described patient population
