Job Title: Registered Practical Nurse BSO

About the Role:

We are seeking a highly motivated and experienced Registered Practical Nurse to join our Behavioural Support Team (BSO) as a Registered Practical Nurse BSO. In this critical role, you will provide clinical leadership and expertise to our team, working closely with residents, families, and healthcare professionals to deliver high-quality care and support.

Key Responsibilities:
  • Provide sound clinical knowledge and skills to support residents with responsive behaviours
  • Collaborate with the Behavioural Support Team to develop and implement care plans
  • Lead and support LTC home staff in the use of frameworks such as PIECES and UFIRST
  • Maintain a short-term active clinical caseload of older adults presenting with or at risk for responsive behaviours
  • Provide comprehensive bio-psychosocial geriatric assessments, treatment, follow-up and discharge planning
  • Coach and mentor LTC home staff in the implementation of behaviour care plans
  • Aid in the transition of long-term care residents with responsive behaviours between community, hospital and neighbouring long-term care homes
  • Collaborate and communicate with healthcare professionals to provide wraparound care
  • Convene and chair regular Behaviour Rounds to discuss care plans with internal and external resources
Requirements:
  • Registered Practical Nurse with current registration with the College of Nurses of Ontario
  • Current CPR/First Aid certification
  • Training in Gentle Persuasive Approaches (GPA)
  • Minimum one (1) year experience with clients in long-term care or residents in a long-term care home setting
  • Demonstrated knowledge and skill working with older adults presenting with or at risk for responsive behaviours
  • Demonstrated ability to train, coach, and mentor others
  • Demonstrated ability to take professional accountability for leading, organizing, providing and delegating care activities for residents and families
Preferred Qualifications:
  • U-FIRST, P.I.E.C.E.S., Dementiability
  • Basic computer skills – MDS/RAI, Microsoft Word, Excel and online learning utilities
  • Experience with quality improvement methods and processes
