Position: Outreach Program Nurse Practitioner

Contract Duration: 7 Months

Hamilton Urban Core is a prominent urban health center dedicated to delivering exceptional primary health services and health promotion initiatives. Our extensive health offerings are provided by a vibrant team of multidisciplinary health professionals. Serving diverse populations, we focus on individuals and families facing chronic and complex health and social challenges, particularly those experiencing poverty.

The Nurse Practitioner will be an integral part of our collaborative health team, delivering holistic and comprehensive primary healthcare services to individuals, families, and communities in an urban setting. As a Registered Nurse in the Extended Class, recognized by the Ontario College of Nurses, you will employ advanced assessment techniques, physical examinations, and therapeutic skills to provide treatment, counseling, or referrals to clients. Utilizing a cooperative care model, the Nurse Practitioner will engage with other healthcare professionals in health promotion and disease prevention, which may encompass individual, group, or community interventions.

Compensation: $106, $122,178.06

Ideal Candidate Qualifications:

  • Bachelor of Science in Nursing (BScN) with Nurse Practitioner certification and registration with the Ontario College of Nurses in Extended Class
  • Comprehensive nursing experience
  • Strong clinical, assessment, and problem-solving abilities
  • Excellent interpersonal and communication skills
  • Interest and experience in primary healthcare, ideally within a community health context, and as part of a multidisciplinary team
  • Awareness and sensitivity to issues affecting urban populations, including immigrants, isolated seniors, street youth, individuals living in poverty, those with mental health challenges, addiction issues, and those at risk of homelessness
  • Familiarity with community health centers
