Job Title: Rehabilitation Service Worker

We are seeking a skilled and compassionate Rehabilitation Service Worker to join our team at Mountains and Meadows Care Group. As a key member of our rehabilitation team, you will play a vital role in supporting individuals with mental health needs to achieve their goals and improve their overall well-being.

Job Summary:

The Rehabilitation Service Worker will work collaboratively with our multidisciplinary team to provide individualized support and services to clients. This will include assessing client needs, developing and implementing rehabilitation programs, and providing ongoing support and guidance to clients and their families.

Key Responsibilities:
  • Assess client needs and develop individualized rehabilitation plans
  • Implement and evaluate rehabilitation programs
  • Provide ongoing support and guidance to clients and their families
  • Collaborate with the multidisciplinary team to ensure comprehensive care
  • Document client progress and maintain accurate records
Requirements:
  • Developmental Services Worker (DSW) Training
  • Mandt System training
  • Mental Health First Aid Certificate
  • Non-Violent Crisis Intervention (NVCI) Certificate
  • Medication Administration Certificate
  • First Aid Certificate
  • Food Safety Certificate
