Patient Care Coordinator

We are seeking a highly skilled and experienced Registered Nurse to join our team as a Patient Care Coordinator. This is a critical role that requires strong clinical leadership and coordination skills to ensure the delivery of high-quality patient care.
Key Responsibilities- Provide clinical supervision and guidance to nursing staff, ensuring that patients receive high-quality care and that nursing practices are evidence-based.
- Coordinate patient assignments and monitor staff workload to ensure that patients receive timely and effective care.
- Develop and implement plans for patient discharge and referral, ensuring that patients receive the necessary support and resources to achieve optimal outcomes.
- Collaborate with other healthcare professionals to promote interprofessional collaboration and team building, and to support the development of clinical skills and knowledge.
- Participate in quality improvement and risk management activities, and contribute to the development of policies and procedures that promote safe and effective patient care.
- Provide input into the development of learning objectives and provide clinical support and guidance to staff, ensuring that they have the necessary skills and knowledge to provide high-quality patient care.
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience.
- Completion of post-graduate coursework and/or certification applicable to the designated clinical area.
- Clinical proficiency/expertise in the designated clinical area.
- Ability to supervise and provide leadership and work direction.
- Ability to coordinate patient assignments, in collaboration with staff.
- Ability to monitor staff workload and patient factors and adjust patient assignments accordingly.
