Job Title: Mental Health Support Worker

We are seeking a skilled Mental Health Support Worker to join our team at Heritage Nanny Community Living and Support Inc. The successful candidate will provide support and services to individuals with mental health needs, promoting their independence and well-being.

Key Responsibilities:
  • Prepare reports and case histories to document client progress and needs
  • Perform administrative tasks, including data entry and record-keeping
  • Assess client needs and develop specialized rehabilitation programs to support their goals
  • Provide suicide and crisis intervention, as well as conflict resolution and mediation
  • Supervise activities of clients living in group homes and half-way houses, ensuring their safety and well-being
  • Liaise with other social services agencies and healthcare providers to coordinate client care
  • Develop and implement life skills workshops and training programs to support client independence
  • Conduct individual and group counselling sessions to promote client well-being
  • Establish and maintain relationships with clients, families, and community partners to ensure effective support and services
Requirements:
  • 2 years of experience in a related field, preferably in mental health support or rehabilitation services
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Ability to work independently and as part of a team, with a strong commitment to client-centered care
  • Knowledge of mental health principles, practices, and legislation, as well as community resources and services
  • Ability to maintain confidentiality and handle sensitive information with discretion
