VON Canada is seeking a dynamic and compassionate Registered Nurse to join our Visiting Nursing team. As a key member of our team, you will provide high-quality, client-centered care to individuals and families in their homes and community.
About the Role:- Provide comprehensive nursing care to clients, including physical assessments, symptom control, palliative care, rehabilitation therapy, and wound care.
- Develop and implement individualized care plans in collaboration with clients, families, and caregivers.
- Conduct thorough assessments of clients' physical, emotional, and social needs, and develop strategies to address these needs.
- Collaborate with other healthcare professionals to ensure seamless care and support for clients.
- Provide education and support to clients and families to promote independence and self-care.
- Participate in quality improvement initiatives to enhance the delivery of care and services.
- Manage and coordinate care for clients with complex needs.
- Provide cost-effective, organized, and outcome-directed care.
- Develop and implement care plans that address clients' physical, emotional, and social needs.
- Provide education and support to clients and families to promote independence and self-care.
- Collaborate with other healthcare professionals to ensure seamless care and support for clients.
- Participate in quality improvement initiatives to enhance the delivery of care and services.
- Current registration with the provincial nursing regulatory body.
- Canadian Nurses Association certification in Community Health Nursing an asset.
- Experience in community health an asset.
- Strong written and verbal communication skills.
- Ability to work independently and as part of a team.
- Ability to problem-solve and adapt nursing interventions as needed.
- Ability to maintain confidentiality and adhere to strict confidentiality standards.
This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying, bending, reaching, kneeling, and other movements that emphasize good body mechanics.
