Job Summary

We are seeking a highly motivated and organized Peritoneal Dialysis Registered Nurse (PD RN) to join our team at TRC Total Renal Care Inc. As a PD RN, you will be responsible for providing comprehensive patient care in an independent setting, educating patients on peritoneal dialysis, and empowering them to take control of their health.

Key Responsibilities
  • Provide comprehensive patient care in an independent setting, including assessing, diagnosing, and implementing complete care plans.
  • Teach patients and their care partners how to administer peritoneal dialysis and manage their treatment.
  • Track and organize patient cases, ensuring timely and effective care.
  • Work independently, building deep relationships with patients, support systems, and care partners.
  • Collaborate with healthcare teams to ensure seamless care coordination.
Requirements
  • Current Registered Nurse (RN) license in the state of practice.
  • Minimum of 9 months of RN experience.
  • Current driver's license, reliable transportation, and vehicle insurance in state of residence.
  • Associate degree in nursing (A.D.N) from accredited school of nursing required.
  • Basic computer skills and proficiency in MS Word and Outlook.
Preferred Qualifications
  • Bachelor of Science in Nursing (B.S.N); three-year diploma from accredited diploma program may be substituted for nursing degree.
  • Previous Chronic Hemodialysis, Acute Dialysis, Peritoneal Dialysis (PD), or Home Hemodialysis (HHD) nursing experience.
  • Training/Teaching/Healthcare Education background.
  • Previous case management experience.
  • Familiarity with, or excitement to learn about telehealth technologies.
  • Certified Nephrology Nurse (CNN), Certified Dialysis Nurse (CDN).
