We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Sunrise Senior Living. As a Wellness Nurse, you will play a vital role in ensuring the overall well-being of our residents, providing high-quality care and services that promote their physical, emotional, and social health.
Responsibilities- Resident Care: Identify ongoing needs and services of residents, ensuring the highest quality resident services are delivered.
- Healthcare Services: Ensure proper follow-through, participate in evaluations, and document changes in healthcare services and status.
- Resident Evaluations: Assist the Resident Care Director in conducting resident evaluations and gathering data at move-in, 30 days post-move-in, semi-annually, and with any change in status.
- Wellness Visits: Conduct monthly wellness visits for all residents, monitoring their health and well-being.
- Incident Reporting: Monitor and follow up with residents who have experienced falls, accidents, or incidents, documenting and reporting as required.
- Communication: Maintain open communication with residents' families and responsible parties to ensure medical needs are met.
- Medical Support: Contact attending physicians when necessary and/or upon family request.
- Medical Supplies: Maintain medical supplies and emergency kits for the community.
- Team Support: Provide clinical support and assistance to community team members as needed.
- Education: Graduate of an approved college or school of nursing.
- Licensure: Current BCCNM registration and a valid state license as a professional LPN/LVN.
- Experience: Minimum of one year of experience in home health, assisted living, hospital, or long-term care environments.
- Skills: Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, as well as federal, state, and local regulations.
- Communication: Possesses written and verbal skills for effective communication and the ability to teach and guide team members to provide quality clinical care.
