Nurse - Primary Care Team

Vancouver Coastal Health is seeking a Registered Nurse (RN) or Registered Psychiatric Nurse (RPN) to join the Primary Care/Integrated Care Team at Raven Song Community Health Centre in Vancouver, BC.
Key Responsibilities- Work collaboratively with the interdisciplinary team to coordinate and develop a plan of care for clients with complex health needs.
- Provide direct care activities, including assessments, planning, implementation, and evaluation of client care.
- Act as a resource to the team, providing information and nursing support based on nursing theory and practice.
- Establish and maintain therapeutic relationships with clients and their families.
- Provide consultation and problem-solving support to the team as needed.
- Current practicing registration as a Registered Nurse (RN) or Registered Psychiatric Nurse (RPN) with the British Columbia College of Nurses and Midwives (BCCNM).
- One (1) year of recent related experience or completion of a full final clinical placement in a community setting.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
