About the Role

The Licensed Practical Nurse (LPN) is a key member of our care team, responsible for coordinating care delivery and ensuring the highest quality of life for our residents. As a LPN, you will be the role model and team leader for an assigned community, working closely with the Director of Care and other healthcare professionals to develop and implement individualized care plans.

Responsibilities
  • Assess residents' needs and develop comprehensive care plans that meet their unique requirements.
  • Liaise with physicians, families, and other healthcare professionals to ensure seamless care delivery.
  • Administer medications and treatments within the scope of your practice, and monitor residents' physical and psychological conditions.
  • Participate in Interdisciplinary Care Conferences and contribute to the development of care plans.
  • Support the administrative requirements of the community, including admissions and discharge processes.
  • Provide coaching and direction to care staff, and participate in orientation and training programs.
  • Maintain up-to-date knowledge of community policies and procedures, and perform other related duties as required.
Qualifications and Skills
  • Graduate of an approved program for Practical Nursing and holds a current license with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.
  • Valid Occupational First Aid Level 1 certification.
  • Successful annual Resident Assessment Instrument (RAI) competency testing.
  • Previous experience in a long term care facility is an asset.
  • Ability to read, write, and speak English proficiently, and understand verbal and written instructions.
  • Computer skills, including the ability to navigate basic computer programs such as Word, Excel, and Outlook.
  • Ability to push, pull, stoop, crouch, lift, and carry up to 25 lbs, and work with cleaners, detergents, and other cleaning equipment.
