Role Summary: The Clinical Coach at VON Canada plays a pivotal role in facilitating practice transformation to enhance access to high-quality clinical services in accordance with the Palliative Care Health Services Delivery Framework and the Ontario Palliative Care Competency Framework.

Primary Responsibilities:

  • Ensure community initiatives align with directives from the Ontario Palliative Care Network
  • Champion equity, inclusion, and diversity within Ontario's healthcare landscape
  • Deliver coaching and mentorship on palliative care practices to staff and healthcare professionals in community settings
  • Collaborate with leadership on local service integration and change management strategies
  • Provide expert consultation on clinical care for patients with complex needs
  • Lead engagement initiatives with partner organizations
  • Support Indigenous communities in their implementation efforts
  • Work with regional stakeholders on the execution of the Delivery Framework
  • Contribute to educational and system integration projects
  • Keep regional teams informed about community challenges and advancements
  • Assist in data collection for evaluation and reporting purposes
  • Communicate key messages tailored for various partner groups
  • Support planning efforts for Ontario Health Teams and palliative care models
  • Provide educational resources to community partners regarding palliative care
  • Assess and guide quality improvement initiatives within community organizations
  • Mentor healthcare providers and spearhead practice transformation projects

General Responsibilities:

Qualifications and Experience:

  • Degree in a recognized healthcare discipline
  • At least five years of experience in palliative care
  • Experience in research, program evaluation, and quality enhancement initiatives
  • Familiarity with various healthcare roles and environments
  • Expertise in palliative care consultation and regional healthcare priorities

Required Skills:

  • Clinical coaching, consultation, and analytical thinking abilities
  • Capacity to perform under pressure and collaborate within a team
  • Proactive problem-solving and negotiation skills
  • Strong communication and relationship-building capabilities
  • Knowledge of educational technologies and patient-centered care
