Licensed Practical Nurse, Community Outreach Specialist

We are seeking a skilled Licensed Practical Nurse to join our multidisciplinary mental health services team as an LPN Outreach Team Member. In this role, you will work collaboratively with RNs and RPNs to provide personal and nursing care services to clients with a variety of mental health disorders.
Key Responsibilities- Medication Administration: Administer oral, subcutaneous, and intramuscular medications in accordance with pharmaceutical and organizational policies and procedures.
- Client Assessments: Assess and monitor clients' actual and potential strengths and limitations, including physiological, psychological, socio-cultural, and spiritual needs on an ongoing basis.
- Client Care Planning: Contribute to the development and modification of clients' plans of care, attending and providing input into interdisciplinary care planning meetings.
- Client Prioritization: Plan, organize, and establish client care plan priorities when providing care, responding to unexpected events and changing client needs within scope of practice and competency guidelines.
- Nursing Care: Provide nursing care to clients, both in their homes and in the community, according to identified policies and procedures, technology, and Fraser Health standards, evaluating outcomes, recording observations, and reporting problems and/or changes to designated staff.
- Documentation: Document and update relevant care and procedures information to ensure patient safety.
- Client Support: Assist clients with activities of daily living, such as serving meals and feeding, client transports, bathing, dressing, skin care, toileting needs, and overseeing client exercise routines, as applicable.
- Quality Improvement: Participate in quality improvement and program activities to maintain a consistent level of care and ensure that interdisciplinary team members work collaboratively.
- Safe Environment: Maintain a safe working environment for clients and others by adhering to established safety policies, reporting unsafe conditions and/or malfunctioning equipment.
- Staff Orientation: Assist in the orientation and education of new staff, students, and other members of the interdisciplinary team, in collaboration with the Manager or designate.
- Education and Experience: Graduation from a recognized program of Practical Nursing or an equivalent combination of education, training, and experience.
- Registration and Certifications: Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM), current First Aid, CPR, and Food Safe certificates.
- Competency Education: Completion of or eligible to complete MHSU Competency Education.
- Driver's License: Valid BC Driver's License and access to a personal vehicle with appropriate insurance coverage.
- Skills and Abilities: Ability to communicate effectively, both verbally and in writing, physical ability to carry out the duties of the position, ability to work independently and in cooperation with others, ability to operate related equipment, ability to organize and prioritize, ability to establish and maintain rapport with clients, and ability to observe and recognize changes in client condition.
