Licensed Practical Nurse, Outreach Specialist

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team in Abbotsford, B.C. as a Relief Part Time Licensed Practical Nurse, Outreach. As a member of our multidisciplinary mental health services team, you will work collaboratively with RNs and RPNs to provide personal and nursing care services to clients with a variety of mental health disorders.
Key Responsibilities- Administer oral, subcutaneous and intramuscular medications in accordance with pharmaceutical and organizational policies and procedures.
- Assess and monitor the patient's actual and potential strengths and limitations, including physiological, psychological, socio-cultural and spiritual needs on an ongoing basis.
- Provide a supportive environment for the client, contribute to the development and modification of client's plan of care, and attend and provide input into interdisciplinary care planning meetings.
- Plan, organize and establish client care plan priorities when providing care, respond to unexpected events and changing client needs within scope of practice and competency guidelines.
- Provide nursing care to clients, both in their homes and in the community, according to identified policies and procedures, technology and Fraser Health standards, evaluate outcomes, record observations and report problems and/or changes to designated staff.
- Graduation from a recognized program of Practical Nursing or an equivalent combination of education, training and experience.
- Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Current First Aid, CPR, and Food Safe certificates.
- Completion of or eligible to complete MHSU Competency Education.
- Valid BC Driver's License and access to a personal vehicle with appropriate insurance coverage.
