Community Support Worker
We are seeking a highly motivated and compassionate Community Support Worker to join our team at Discovery Point - Fayetteville. As a Community Support Worker, you will play a vital role in providing support services to individuals with developmental disabilities and their families.
Key Responsibilities:
- Act as the initial point of contact for individuals requesting services, representing their needs and completing intake assessments.
- Collaborate with a multi-disciplinary team to develop and implement individualized support plans.
- Assist individuals and families in accessing community resources and government services.
- Provide support and guidance to individuals with developmental disabilities, promoting independence and self-advocacy.
Requirements:
- A Social Service Worker Diploma or Social Worker Degree, or proof of registration with the Ontario College of Social Workers and Social Service Workers.
- A valid Class G driver's license and use of a vehicle.
- Double COVID-19 vaccination.
Working Conditions:
- 75 hours per 2 weeks.
Compensation:
- $63 per hour.
