Care Coordinator Specialist

At Home and Community Care Support Services, we are seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs, providing access to community services, and engaging with patients, caregivers, and healthcare practitioners.
Key Responsibilities:- Collaborate with patients and their families to assess care needs and develop individual care plans
- Link patients with service providers and coordinate care plan delivery
- Establish a helping relationship with patients and their families, balancing their needs and choices with available resources
- Utilize excellent interpersonal, communication, assessment, problem-solving, and decision-making skills to ensure patient-centered care
- Membership in good standing with the College of Nurses of Ontario
- Recent experience in community health or a related field
- Knowledge of the healthcare delivery system and community resources
- Excellent time management, prioritization, and organizational skills
- Ability to accurately complete required documentation, reports, and forms
- Valid driver's license and access to a reliable vehicle
- Proficient in a Windows environment
- Advanced oral and written proficiency in English and French
- Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatrics
- Case management experience or recent related community experience
- Ability to speak French or another second language
