Qualifications and Requirements

We are seeking a highly skilled and experienced Long Term Care Case Manager to join our team at Island Health. The ideal candidate will possess a Baccalaureate degree in Social Work from a recognized university and be currently registered with the BC College of Social Workers.

Additionally, the successful candidate will have post-degree preparation in gerontology or a related field, as well as three years of recent experience in gerontology, including two years of community health experience. A valid BC Driver's license is also required.

Key Responsibilities
  • Apply the social work process as required, adhering to the BC College of Social Workers Practice Standards and Code of Ethics.
  • Demonstrate competence in social work practice areas such as chronic disease management, health promotion, and illness prevention.
  • Work effectively as part of an inter-professional healthcare team, collaborating with other staff, programs, and organizations, as well as with clients and their caregivers/families.
  • Work independently and as a member of a team, utilizing sound judgment, good observation and assessment skills, tact, and empathy.
  • Resolve conflicts, problem-solve, and build consensus, fact-find, seek out information and/or resources, and intervene in crisis or difficult situations.
  • Demonstrate a working knowledge of applicable legislation and maintain a commitment to continuing professional development as required by the employer.
  • Foster and promote good public relations, communicate effectively, both verbally and in writing, and deal with others effectively.
  • Operate related equipment, including computers and software, and be physically and emotionally able to carry out the duties of the position.
  • Use a personal vehicle for work-related purposes.
