**Key Responsibilities**
- Provide comprehensive primary care to patients, including routine check-ups, health assessments, and management of chronic conditions
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication
- Participate in quality improvement initiatives to enhance patient outcomes and clinic operations
