About the Role

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Fraser Health Authority. As an LPN, you will play a vital role in providing high-quality care to our patients with mental health and substance use issues.

Key Responsibilities
  • Establish therapeutic relationships with clients and families, assessing their needs and developing individualized care plans.
  • Provide direct care to stable and predictable clients, including medication administration, wound care, and vital sign monitoring.
  • Collaborate with interdisciplinary teams to develop and implement care plans, ensuring seamless transitions and continuity of care.
  • Conduct risk assessments and develop strategies to mitigate potential harm, promoting a safe and supportive environment for clients.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices and policies.
Requirements
  • Graduation from a recognized Practical Nursing program.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • One year recent, related experience in substance use and/or mental health care setting, or an equivalent combination of education, training, and experience.
  • Current certificates in CPR Level C or equivalent and First Aid.
