Diabetes and Chronic Disease Management Dietitian

About the Opportunity
Reporting to the Health Services Manager, Primary Health Care, Chronic Disease Management and Wellness, you will provide expert knowledge and skills on a wide range of issues from advocacy to assessment to case management. As a Diabetes care specialist, you will use critical thinking and clinical judgment in coordinating, facilitating, and improving the management of patients with Diabetes and co-occurring chronic conditions within an integrated primary health care model.
You will provide comprehensive and coordinated care for individuals with Diabetes, consistent with DCPNS and CDA standards and guidelines. As a member of a primary health care (PHC) Diabetes Centre team, you will deliver a range of PHC services including, but not limited to, health promotion, illness prevention, screening, treatment, and monitoring of people living with diabetes. Expertise may also be required to support people living with other chronic health problems, community development, and advocacy.
You will collaborate with others to increase community capacity as an educator, researcher, and clinical practitioner. These services are provided within a holistic, client/family-centered care framework.
About You
- You possess a Bachelor of Science in Human Nutrition from a recognized university.
- You have completed a Dietetic internship (a 1 year integrated or post graduate program).
- You have 1 year of diabetes chronic disease management experience. If no applicants meet the one-year experience required, applicants with less than one year's experience may be considered.
- You are a current member of the Nova Scotia Dietetic Association.
- You maintain your Nova Scotia license through annual continuing education.
- Two years of clinical experience is preferred.
- You have an interest in care for chronic pain.
- Membership in Dietitians of Canada is preferred.
- Current CPR/BLS certification is required.
- You are designated as a Certified Diabetes Educator or are required to complete this within 2 years.
- You are willing to obtain adult insulin dose adjustment within 6-12 months and insulin dose adjustment for pregnancy within 2 years.
- You have experience with group education.
