Continuing Care Coordinator

The Care Coordinator is responsible for assessing, planning, and authorizing services for clients referred to Continuing Care. They help provide an array of services and programs to assist individuals and families cope with complicated health situations in the community.
The Care Coordinator assists clients to identify their goals, unmet needs, and resource requirements. Based on the assessment, they formulate a plan to meet identified goals and work with interprofessional teams to facilitate hospital discharge to return to the community.
Care Coordinators work in the community, hospital, or long-term care facilities, maintaining ongoing communication with clients, family, caregivers, care providers, and healthcare teams within the client's identified circle of care. They are responsible for reassessments at routine intervals and skilled care management.
About YouWe are seeking a qualified professional with a Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies. The ideal candidate will have current and ongoing registration with the applicable Nova Scotia Professional Regulatory body and a minimum of three years' experience in a community or long-term care setting.
The successful candidate will possess excellent assessment, interviewing, and negotiation skills, as well as excellent verbal and written communication skills. They will be able to work independently and cooperatively in a busy multidisciplinary situation, prioritizing workload and managing time efficiently.
Hours- Full-time, long-assignment; 70 hours bi-weekly
- Commencing September 2024; ending August 2025
- Shifts may include days, evenings, weekends, and holidays
$41.06 - $46.75 hourly
