About the Role

VHA Home HealthCare is seeking a highly skilled and organized Health Services Coordinator to join our team. As a Health Services Coordinator, you will play a critical role in ensuring the smooth delivery of healthcare services to our clients.

Key Responsibilities
  • Client Scheduling: Coordinate client schedules, ensuring timely and efficient delivery of services.
  • Service Coordination: Work closely with healthcare teams, including Personal Support Workers, Nurses, and Rehab Service Providers, to ensure seamless service delivery.
  • Communication: Maintain regular communication with clients, families, funders, and healthcare teams to ensure client needs are met.
  • Service Planning: Develop and maintain client service plans, ensuring alignment with client goals and needs.
  • Administrative Tasks: Perform administrative tasks, including data entry, to support the efficient operation of the team.
Requirements
  • Education: Post-secondary school education, preferably in healthcare education.
  • Experience: Minimum of three years of related work experience in a healthcare organization or agency, or in an administrative position requiring extensive scheduling.
  • Skills: Excellent telephone skills, strong organizational skills, attention to detail, and ability to prioritize and multitask.
  • Knowledge: Knowledge of the GTA and TTC routes or relevant contracted areas.
  • Computer Skills: Strong computer and data entry skills, with the ability to input data at a speed of 50 wpm.
