Healthcare Social Worker

The Health Care Social Worker is a vital member of the interdisciplinary healthcare team, providing collaborative, holistic care on the primary to quaternary care continuum. This role focuses on the relationships between individuals and their environments, which are integral to psychosocial, emotional, and spiritual well-being.
Key Responsibilities- Engage with patients/clients to build upon their strengths, focusing on existing coping strategies and problem-solving skills, past successes, and personal resources.
- Conduct biopsychosocial assessments to inform care planning and intervention.
- Develop and implement evidence-based treatment approaches for individuals with severe and persistent mental illness, such as Psychosocial Rehabilitation and Motivational Interviewing.
- Collaborate with community partners to provide comprehensive care and support.
- Communicate effectively with patients/clients, families, and healthcare teams to ensure seamless care coordination.
- Bachelor's degree in Social Work from a recognized university.
- Current registration with the Nova Scotia College of Social Workers.
- Two (2) years post BSW related clinical experience required.
- Knowledge and understanding of the Diagnostic & Statistical Manual of Mental Disorders.
- Previous experience working in adult community mental health preferred.
