Healthcare Social Worker

The Health Care Social Worker is a vital member of the interdisciplinary healthcare team, providing collaborative and holistic care across the primary to quaternary care continuum. This role focuses on the relationships between individuals and their environments, which are integral to psychosocial, emotional, and spiritual well-being. The social determinants of health are embedded in social work theory and practice, and the dynamic and interactive process of engagement, assessment, planning, intervention, and evaluation is a key aspect of this position.
About YouWe are seeking a highly skilled and experienced Social Worker to join our team. The ideal candidate will have a Bachelor's degree in Social Work from a recognized university, current registration with the Nova Scotia College of Social Workers, and at least two years of post-BSW clinical experience. A strong understanding of the Diagnostic & Statistical Manual of Mental Disorders and previous experience working in adult community mental health are highly desirable. The successful candidate will demonstrate a client-/family-centered approach, familiarity with various evidence-based treatment approaches, and excellent communication skills. Additionally, the ability to complete biopsychosocial assessments, work collaboratively with community partners, and demonstrate effective time management and disciplined self-organization are essential.
Key Responsibilities- Provide collaborative and holistic care to patients/clients across the primary to quaternary care continuum
- Focus on the relationships between individuals and their environments, which are integral to psychosocial, emotional, and spiritual well-being
- Embed social determinants of health in social work theory and practice
- Engage in dynamic and interactive processes of engagement, assessment, planning, intervention, and evaluation
- Work collaboratively with community partners to provide comprehensive care
- Complete biopsychosocial assessments and develop effective treatment plans
- Demonstrate excellent communication skills and a client-/family-centered approach
Nova Scotia Health Authority offers a competitive salary range of $33.96 - $42.89 hourly, as well as a comprehensive benefits package, including health, dental, travel, long-term disability, and life insurance coverage, and a defined benefit pension plan. Successful candidates may also be eligible for a $10,000 signing bonus and up to $5,000 relocation assistance with a defined Return of Service contract.
