About the Role

We are seeking a highly skilled and experienced Clinical Pharmacist to join our team at Markham Stouffville Hospital. As a Clinical Pharmacist, you will play a vital role in providing clinical pharmacy services to patients across our hospital sites.

Key Responsibilities
  • Provide clinical pharmacy services to patients in various areas, including surgery, medicine, ICU/CCU, childbirth and children, psychiatry, rehabilitation, palliative, complex care, oncology, and emergency.
  • Work collaboratively with the inter-professional team to optimize medication therapy and safety.
  • Participate in patient education, support teaching students, and provide drug information to nursing and medical staff.
  • Contribute to the development and implementation of pharmacy policies and procedures.
Requirements
  • Graduated from an accredited university pharmacy program.
  • Licensed under Part A registration status with the Ontario College of Pharmacists.
  • Hospital pharmacy residency is preferred.
  • Previous experience in a hospital setting with unit-dose and IV admixture programs is required.
  • Demonstrated experience identifying, resolving, and preventing drug-related problems to optimize medication therapy and safety.
  • Demonstrated computer efficiency is essential.
  • Excellent communication and interpersonal skills and the ability to work collaboratively with the members of the interdisciplinary team.
  • Evidence of professional development, continuing education, and leadership experience is preferred.
  • Demonstrated good attendance and performance records with the ability to maintain these same standards.
  • Demonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital-related policies, health and safety legislation, and best practices and completing relevant mandatory education as required.
What We Offer

We offer a competitive salary range of $93,346.50 - $122,772 per year, as well as a comprehensive benefits package and opportunities for professional growth and development.
