About the Role

We are seeking a highly skilled and compassionate Outreach Support Specialist to join our team at Fraser Health Authority. As a key member of our multidisciplinary team, you will provide community-based support services to individuals experiencing mental health, behavioral, and developmental challenges.

Key Responsibilities
  1. Client Assessment and Support: Observe, interview, and assess client behavior to identify problems and needs, soliciting client feedback, inquiries, and complaints; respond to client needs as appropriate.
  2. Independence and Self-Sufficiency: Encourage and promote independence and self-sufficiency for clients and their caregivers/families; develop short-term strategies to deal with various situations.
  3. Treatment Plan Development: Provide feedback and suggested recommendations to the treatment plans within established guidelines.
  4. Documentation and Record-Keeping: Document the care requested and progress made in the communications books; maintain client records.
  5. Team Collaboration: Report regularly to the multidisciplinary team on the condition, behavior, performance, progress, and care needs of clients and their caregivers/families.
  6. Life Skills Training: Provide support services, coaching, and behavior modeling for the client and their caregivers/families to assist with the development of physical, social, emotional, and life-skills, including all activities of daily living, interpersonal skills, meal planning and preparation, and household and budget management.
  7. Community Partnerships: Consult and work cooperatively with community agencies and families to maintain coordination in the service and treatment process.
  8. Advocacy: Act as an advocate for the client in dispute situations by responding to and/or coordinating response to problems regarding landlord-tenant issues, financial assistance, or accessing community resources and housing, and developing workable solutions that meet the needs of the client.
  9. Medication Management: Observe and support individuals in appropriate medication management, in accordance with the plan of treatment/care and established policies and procedures.
  10. Emergency Response: Observe and monitor clients in accordance with plan of treatment/care, assess and respond in potential emergency or crisis situations in accordance with established procedures, and provide feedback and recommendations to the treatment/care plan.
  11. Client Progress: Document client progress to maintain permanent record of client activity according to established methods and procedures.
  12. Substance Use Treatment: Participate in providing substance use treatment, home finding and general housing support, individual and personal care services.
  13. Vocational Support: Provide individual vocational supportive counseling and other work-related supportive services.
  14. Community Resource Awareness: Maintain an awareness of community resources, including health care services, social, economic, recreational, employment, and educational services and resources, and other agencies and share that information as required.
  15. Client Transportation: Facilitate client attendance at appointments, programs, and services, including escorting or accompanying client to appointments, and for shopping and leisure activities, including driving a vehicle.
  16. Home Maintenance: Teach, demonstrate, model, and support home maintenance and household management methods, and provide basic information to clients related to activities of daily living, e.g. housekeeping, yard maintenance, meal planning, and preparation, grocery shopping, and managing monthly living expenses.
  17. Workplace Safety: Ensure a safe and healthy working environment by observing universal precautions and infection control procedures, removing obvious hazards, reporting faulty equipment, accidents, injuries, and near misses, and adhering to and enforcing rules regarding safety.
  18. Professional Development: Participate in staff meetings, quality improvement initiatives, committees, and in-services, and maintain up-to-date knowledge through involvement in work-related continuing education, as required.
  19. Team Support: Provide direction to volunteers and assist with orientation of new staff and education experiences of students, as well as sharing expertise with other members of the team, as required.
Requirements

We are seeking a candidate with a Grade twelve (12) plus successful completion of the Community Mental Health Worker Certificate or Social Services Worker Diploma and two (2) years' recent related experience, or an equivalent combination of education, training, and experience. Current CPR certificate and valid Class 5 BC Driver's License are also required.

The ideal candidate will possess strong communication and interpersonal skills, with the ability to work effectively with diverse populations and teams. They will also have a strong understanding of legislation related to mental health and social services, as well as knowledge of available programs and services in the community.
