Community Mental Health Support Specialist

As a Community Mental Health Support Specialist with Fraser Health, you will play a vital role in providing outreach community-based support services to individuals experiencing mental health, behavioral, and developmental problems. Reporting to the Care Coordinator, you will work as part of a multidisciplinary team to provide life skills training, coaching, advocacy, and support to clients and their caregivers/families.
Key Responsibilities
Observe, interview, and assess client behavior to identify problems and needs
Provide feedback and recommendations to treatment plans within established guidelines
Document client progress and maintain permanent records
Participate in providing substance use treatment, home finding, and general housing support
Facilitate client attendance at appointments, programs, and services
Teach, demonstrate, model, and support home maintenance and household management methods
Requirements
Grade twelve plus successful completion of the Community Mental Health Worker Certificate or Social Services Worker Diploma
Two years' recent related experience, or an equivalent combination of education, training, and experience
Current CPR certificate
Valid Class 5 BC Drivers Licence and access to personal vehicle with appropriate business insurance coverage
