Job Summary

Fraser Health is seeking a highly skilled and compassionate Outreach Support Worker to join our multidisciplinary team. As a key member of our Mental Health & Substance Use Services team, you will provide community-based support services, assist in mental health and substance use treatment, and promote independence and self-sufficiency for clients and their caregivers/families.

Key Responsibilities
  • Provide outreach community-based support services, including life skills training, coaching, advocacy, and support to clients experiencing mental health, behavioral, and developmental problems.
  • Assist in mental health and substance use treatment, and promote independence and self-sufficiency for clients and their caregivers/families.
  • Observe, interview, and assess client behavior to identify problems and needs, and respond to client needs as appropriate.
  • Encourage and promote independence and self-sufficiency for clients and their caregivers/families, and develop short-term strategies to deal with various situations.
  • Provide feedback and suggested recommendations to treatment plans within established guidelines.
  • Document the care requested and progress made in communications books, and maintain client records.
  • Report regularly to the multidisciplinary team on the condition, behavior, performance, progress, and care needs of clients and their caregivers/families.
  • Provide support services, coaching, and behavior modeling for clients and their caregivers/families to assist with the development of physical, social, emotional, and life skills.
  • Consult and work cooperatively with community agencies and families to maintain coordination in the service and treatment process.
  • Act as an advocate for clients in dispute situations, and develop workable solutions that meet the needs of the client.
  • Observe and support individuals in appropriate medication management, and monitor clients in accordance with the plan of treatment/care.
  • Participate in providing substance use treatment, home finding, and general housing support, and provide individual vocational supportive counseling and other work-related supportive services.
  • Maintain an awareness of community resources, including health care services, social, economic, recreational, employment, and educational services and resources, and share that information as required.
  • Facilitate client attendance at appointments, programs, and services, and participate in and supervise social and recreational activities as required.
  • Teach, demonstrate, model, and support home maintenance and household management methods, and provide basic information to clients related to activities of daily living.
  • Ensure a safe and healthy working environment by observing universal precautions and infection control procedures, and adhere to and enforce rules regarding safety.
  • Participate in staff meetings, quality improvement initiatives, committees, and in-services, and maintain up-to-date knowledge through involvement in work-related continuing education.
  • Provide direction to volunteers and assist with orientation of new staff and education experiences of students, as well as share expertise with other members of the team as required.
Qualifications

Grade twelve plus successful completion of the Community Mental Health Worker Certificate or Social Services Worker Diploma and two years' recent related experience, or an equivalent combination of education, training, and experience. Current CPR certificate. Valid Class 5 BC Drivers Licence and access to personal vehicle with appropriate business insurance coverage.

Knowledge of legislation such as the Mental Health Act and the Guardianship Act. Knowledge of available programs, their interrelationships, and their function in delivering care in the community. Ability to communicate effectively both verbally and in writing. Ability to deal with others effectively. Ability to prioritize and organize client demands made and care needs. Ability to problem-solve. Ability to teach life skills. Sound judgment and good observation skills. Ability to demonstrate an appropriate level of initiative and independence. Ability to work cooperatively as part of a multidisciplinary team. Ability to promote positive change and independence. Physical and ability to carry out the duties of the position. Ability to operate related equipment. Advocacy skills.
