About the Role

The Care Management Specialist plays a crucial role in evaluating, planning, and overseeing the care of clients referred to Continuing Care. This position is dedicated to assisting individuals and families navigate complex health challenges, ensuring they receive the necessary support from the various programs offered by Nova Scotia Health Authority and the Department of Seniors and Long Term Care. The Care Management Specialist collaborates with clients to identify their aspirations, unmet needs, and available resources. Through comprehensive assessments, they help develop tailored care plans aimed at achieving the clients' objectives.

The Care Management Specialist is responsible for authorizing services and facilitating connections to additional professional and volunteer resources, ensuring that community-dwelling clients receive the support they need for as long as possible. They work closely with interdisciplinary teams to ensure smooth transitions from hospital to home.

This role may involve working in various settings, including community environments, hospitals, or long-term care facilities. The Care Management Specialist is accountable for the services and resources they authorize, adhering to the policies and programs set forth by the Department of Seniors and Long Term Care. In unique situations, they advocate for alternative solutions to meet clients' unmet needs. Maintaining ongoing communication with clients, families, caregivers, and healthcare teams is essential, ensuring a cohesive approach to care. The philosophy guiding this role emphasizes maximizing available resources to support clients and families in their home or community. Regular reassessments and proficient care management are also key responsibilities.

Qualifications

Candidates should possess the following qualifications:

  • Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current registration (or eligibility for registration) with the relevant Nova Scotia Professional Regulatory body
  • At least three years of experience with the professional regulatory body
  • Proven experience in community or long-term care settings
  • Familiarity with dedicated case management or discharge planning
  • Proficient in computer use and capable of conducting assessments using a laptop
  • Knowledge of community agencies and available resources
  • Strong assessment, interviewing, and negotiation skills
  • Excellent verbal and written communication abilities
  • Ability to work both independently and collaboratively in a dynamic multidisciplinary environment
  • Strong organizational skills, with the ability to prioritize tasks effectively
  • A valid driver's license and access to a reliable vehicle are essential
  • Proficiency in additional languages is preferred; French is an asset

Work Schedule
  • Permanent Full-Time (70 Hours Bi-Weekly)
  • Shifts may include days, evenings, weekends, and holidays
Compensation and Benefits

The hourly wage is $48.39.

Successful candidates may qualify for a benefits package that includes health, dental, travel, long-term disability, and life insurance coverage, along with a defined benefit pension plan.
