Job Title: MRP Transition Coordinator

At Shared Health, we are seeking a highly skilled and dedicated individual to fill the role of MRP Transition Coordinator. This position plays a critical role in facilitating the transition of renal patients from Renal Health clinics to renal replacement therapies, as well as between various renal replacement therapies.

Key Responsibilities:
  • Coordinate patient transfers between healthcare facilities and renal units, ensuring seamless transitions and optimal utilization of resources.
  • Develop and maintain effective working relationships with patients, families, and interdisciplinary team members to ensure patient-centered care.
  • Participate in the assessment and decision-making processes to ensure effective patient movement and optimal resource utilization.
  • Liaise with various programs, agencies, and facilities to coordinate patient movement, including Indigenous partners and stakeholders.
  • Develop and implement guidelines, protocols, and educational strategies related to patient movement and transition.
  • Provide education and support to renal patients and their families, as well as healthcare providers and the community.
Requirements:
  • Four years of related experience in renal health, peritoneal dialysis, and hemodialysis.
  • Knowledge and experience with electronic health records used in patient transfer processes.
  • Completion of Manitoba Nephrology Nursing Course and Manitoba Indigenous Cultural Safety Training (MISCT) required.
  • Canadian Nurses Association Certification – Nephrology (CNEPH) preferred.

  • Demonstrated ability to work independently and collaboratively with patients, families, and interdisciplinary team members.
  • Ability to establish and maintain effective working relationships and resource contacts.
Working Conditions:
  • Ability to work flexible hours and travel to patients' homes and healthcare facilities.
